Call Passport’s Member Services team toll-free at 1-800-578-0603. We’ll tell you what to expect when you apply for Medicaid. We’ll also tell you the state agency you need to contact and transfer you to get the process started. If the state of Kentucky determines that you’re eligible, you can choose Passport as your health plan!
How much does it cost?
With Passport, your monthly plan premium costs $0, your covered medical services cost $0, and many generic medicines (including over-the-counter medicines) cost $0.
