Passport Health Plan is a provider-sponsored, non-profit, community-based Medicaid health plan serving more than 250,000 people around Kentucky. Recently named the #19 Medicaid health plan in the United States and the top Medicaid plan ranked in Kentucky by the National Committee for Quality Assurance (NCQA) in “NCQA’s Medicaid Health Insurance Plan Rankings 2014–2015,” Passport has been contracted by Kentucky’s Cabinet for Health and Family Services to administer Medicaid benefits since 1997 and has been serving the entire Commonwealth since January 2014.

How can I sign up for Passport Health Plan?
Call Passport’s Member Services team toll-free at 1-800-578-0603. We’ll tell you what to expect when you apply for Medicaid. We’ll also tell you the state agency you need to contact and transfer you to get the process started. If the state of Kentucky determines that you’re eligible, you can choose Passport as your health plan!

How much does it cost?
With Passport, your monthly plan premium costs $0, your covered medical services cost $0, and many generic medicines (including over-the-counter medicines) cost $0.

Is Passport available in my area?
Passport has been serving all 120 Kentucky counties since January 2014, with a network of providers that continues to grow every day.
