Via Christi HOPE

Via Christi HOPE is the Program of All-Inclusive Care for the Elderly (PACE®) provider for Sedgwick County.  There is no program like Via Christi HOPE.  HOPE assists older adults and their loved ones, helping them remain independent and in the community for as long as possible.  This is accomplished through comprehensive, high-quality health and social services tailored to the unique needs of each individual.  HOPE's primary care physicians and interdisciplinary team of professionals provide and coordinate all services, providing "one stop shopping" for program participants.  Comprehensive services include: day center, home care, medical care, medications, medical transportation, social services, rehabilitative services, nutrition counseling and more.  Participants must be at least 55 years old and a resident of Sedgwick County.

Q: What does PACE® stand for?
A: PACE® : Program of All-Inclusive Care for the Elderly
Q: Are there other PACE® providers in the area?
A: Via Christi HOPE is the only PACE® provider for Sedgwick County. The only other PACE® site in Kansas, Midland Care Connection, is in Topeka
Q: What types of payment does Via Christi HOPE accept?
A: Depending on your financial eligibility, payments come from a combination of Medicare, Medicaid, Part D prescription benefit and private pay.
Q: Where is Via Christi HOPE located?
A: The Via Christi HOPE day center is located at the West River Plaza Campus near the intersection of Central and McLean, but services are also provided in the participants' homes.
Q: How long has Via Christi HOPE been in operation?
A: Via Christi HOPE opened in September 2002 and now serves over 200 participants.
